Every student that comes to a Fire Student Ministries event that takes place off
church campus is required to have a completed Medical/Permission form on file.
Below you can download a copy of the Medical Release Form. Please print it out and fill it out as completely
and accurately as you can. You can make the effective dates for as long as the information would be current.
